Policies and FormsMilford Family Practice accepts most insurance plans, but patients should always check with their individual carriers to determine our participation with their plans. If you require a co-payment at the time of your visit, we accept cash, checks and most credit cards.

What to bring to your appointment:

  • Your insurance card
  • Picture identification
  • A list of your current medications
  • Reports pertinent to your visit
  • Medical records (in particular any old operative reports) pertaining to the reason for your visit
